Facility Use Form
In order to utilize a classroom, fellowship hall, sanctuary or other aspects of our facility, you must submit this form to the church office in a timely manner. Until a form is submitted with the deposit enclosed, your reservation cannot be considered.
If for any reason you feel you cannot adhere to the guidelines outlined in this form, then our facility is not available to service your needs and we encourage you to seek another alternative.
We believe that connecting in a smaller, more intimate, group helps to encourage, build up and strengthen our church constituents. We encourage everyone that is a part of our local church to be active in a Connect Group. Currently we have five groups function with the anticipation of adding more in the future. If you are not a part of a Connect Group, please download the form below and submit it to the church office so that we can add you to a group.

This form is for those desiring to either transfer into Pine Grove Church (PGC)
from another church or out of PGC and
to another church.
